DENTAL BENEFITS
The District is a self-insured provider of dental benefits. United Concordia is the Plan Administrator for all dental benefits.
ESPA and Cafeteria and COBRA employees must complete an enrollment form if they want to enroll, withdraw, or make changes to their plan. Other groups are automatically enrolled in the dental plan.
Eligibility and benefits are governed by employee agreements. Benefits summaries for each group's plans are sent out to all employees by email annually during open enrollment and located below. Reach out to Elizabeth Jones at [email protected] or x 4001 with dental plan benefits questions.
VISION BENEFITS
The District's vision insurance is through Davis Vision. Important information about your benefits, contact numbers, and processes are included in the Plan Description document below.
